This phenotype is pathologic
Secondary hyperparathyroidism develops as a result of impaired calcium homeostasis when the failing kidneys disturb the complicated interactions between parathyroid hormone, calcium, phosphorus, and vitamin D.

Chronic renal failure is the most common cause of secondary hyperparathyroidism. Failing kidneys do not convert enough vitamin D to its active form, and they do not adequately excrete phosphorus.
